Who You Are

You are a committed and caring individual eager to support citizens in Cardiff to live independently at home. You possess strong observation and recording skills, a flexible and focused approach, and a willingness to train to the required HSc level. Multilingual abilities are advantageous, especially in Welsh and other community languages. You have a full clean driving license and the use of a car.

What the job involves

As part of The Community Resource Team (CRT) Homecare, you will support clients in achieving their wellbeing goals, preventing unnecessary hospital admissions, and facilitating hospital discharges. You will work alongside health professionals, using a service delivery plan and mobile device to perform daily tasks according to a rota, with possible evening and weekend shifts. Your role requires a permanent 37-hour workweek, subject to enhanced checks and Social Care Wales registration.
