What the Job Involves
The role involves undertaking assessments of risk and care needs for service users and carers, ensuring appropriate actions are taken, monitored, and reviewed. You will manage a varied caseload, engage with clients refusing services, and handle sensitive negotiations. The job requires you to prepare and present reports, maintain high-standard client records, and facilitate needs-led service provisions. You will work closely with Social Services and various external agencies to assess and meet the needs of individuals. Promoting equality, diversity, and rights is integral to the role, alongside team collaboration, developing, and maintaining effective partnerships.
