What the Job Involves
This role involves managing a caseload of clients, providing them with follow-up and monitoring of their mental state, and ensuring that Key Performance Indicators (KPIs) are up to date. You will support carers and facilitate proper coordination with other agencies. Key responsibilities include completing Care Act Assessments, carer assessments, social circumstances reports, Mental Health Act Assessments (MHAA), Mental Capacity Assessments (MCA), and other social work-related engagements and support within the team. Additional duties may be assigned as required by the team.
