Occupational Therapist (Grade J)

Who You Are

We are looking for a skilled and proactive Occupational Therapist to join our Adult Social Care team. You are dedicated to enabling independence and safety for service users through major housing adaptations, manual handling assessments, and the provision of equipment and minor major adaptations. You are committed to working closely with service users, carers, and partner agencies to deliver practical solutions that transform lives.

What the job involves

• Complete comprehensive functional assessments in line with the Care Act.
• Participate in duty triaging and screening/signposting incoming referrals.
• Design and recommend major adaptations like level-access showers, stairlifts, and ramps.
• Prescribe and arrange minor and major equipment tailored to individual needs.
• Conduct manual handling risk assessments and provide moving and handling plans.
• Liaise with contractors, housing teams, and grants officers for adaptation works.
• Maintain accurate records and produce professional reports for funding applications and safeguarding.
• Deliver high-quality, person-centred interventions within agreed timescales.
• Ensure compliance with legislation, safeguarding, and health & safety standards.
• Contribute to service development and continuous improvement.
• Work effectively within a multi-disciplinary team and represent the service professionally.

Skills

• HCPC registered Occupational Therapist
• Experience in community-based social care settings
• Knowledge of housing adaptations and equipment provision
• Understanding of manual handling
• Familiarity with Disabled Facilities Grants (DFG)
• Experience working with contractors and housing teams
• Proficient in using electronic case management systems
