What the job involves
This role involves working collaboratively with individuals to improve and maintain daily living skills, as outlined in their personalized care plans. You will work as part of an integrated team alongside other health and social care professionals to minimize risks and help individuals remain in their homes. Participation in service user assessment and reviews is required, alongside contributing to record maintenance and reporting significant changes. You will collaborate in implementing person-centered support plans and engage with service users in activities. Flexibility is essential to meet evolving service needs, and you will work Monday to Friday from 9 am to 4 pm, following City Council policies and procedures.
