Who You Are
You are a dedicated and experienced social worker with a robust track record of working with adults who have varied needs. You are independent, creative, resilient, and capable of working under pressure. You must be familiar with and effectively practice within statutory guidelines and frameworks. Your approach is proactive and you are able to deliver assessments and support plans within tight schedules. Additionally, you have the ability to work both in-office and remotely with a set schedule, including participating in a duty rota every four weeks.
What the Job Involves
In this role, you will work with the Community Health & Social Care Wanstead & Woodford Cluster, assisting adults from the age of 18 to end of life who have varied needs such as physical disabilities, dementia, Parkinson’s, Autism, Multiple Sclerosis (MS), head injuries, spinal cord injuries, and learning disabilities. The position follows a hybrid working pattern with 2-3 days in the office and the remainder remotely, Monday to Friday from 9 am to 5 pm. Your responsibilities include conducting intense and fast-paced assessments and reviews, which are completed face-to-face, and creating supportive plans and interventions.
