Role

Who You Are

An experienced social worker with a formal social work qualification (CQSW, CSS, DipSW) and a minimum of two years post-qualifying experience. You have a proven track record in assessing and managing risks, designing outcome-focused care plans, and working with older people and their families/carers. You have the ability to use your initiative, are creative and innovative in your approach, and possess a strong commitment to maintaining clients' independence.

What the Job Involves

Reporting to the Team Manager, you will independently manage complex cases involving high levels of risk and safeguarding vulnerable adults. Your role involves conducting community care and safeguarding assessments, developing and implementing care plans, managing caseloads, and ensuring effective service delivery. You will also work collaboratively with clients, their families, and other professionals, maintaining relationships, offering guidance, and resolving conflicts as necessary. Writing reports and potentially giving evidence in court are also part of your responsibilities.
