Who You Are

You are a motivated and registered Social Worker with Social Work England, experienced in delivering high-quality social work to adults and/or carers. You should have a proven track record of working effectively within comprehensive assessment frameworks and handling complex cases while adhering to professional standards and legislative requirements. Your ability to collaborate with multidisciplinary teams and effectively communicate complex information will allow you to deliver exceptional service to the community.

What the job involves

The role involves conducting professional social work assessments for older individuals and adults with specific needs or disabilities. You will safeguard adults and carers at risk, promote wellbeing principles, and support them in maintaining health and independence. Key responsibilities include planning and reviewing care plans, liaising with various agencies for holistic service delivery, conducting Mental Capacity assessments, and promoting the use of direct payments. Your role includes supervising junior staff, promoting continual professional development, and ensuring the service delivers value within budget constraints.
