Who You Are

We are seeking a qualified and experienced Occupational Therapist (OT) to join our vibrant and fast-paced Hospital Discharge Service. You are a proactive and compassionate professional, eager to make a positive impact in the lives of Islington residents. Your focus is on supporting individuals through various stages of the hospital pathway, leveraging their strengths to achieve their desired outcomes.

What the Job Involves

This role is an essential part of Islington's Integrated Discharge Service (IDS), working collaboratively with a team of therapists and a Senior Social Worker. You will assist in assessing risk and crafting interventions to support residents returning home from the hospital. Key responsibilities include:

  • Support the Discharge to Assess pathway by screening referral documentation and determining care plans to safely transition individuals home, participating in a 7-day rota of approximately 2/5 days per week after two months of service.
  • Conduct post-discharge assessments to facilitate therapy interventions that promote independence, recovery, or prevent readmission. Responsibilities include environmental assessments, equipment provision, and creating management strategies for daily activities. Documentation of home visits can typically be completed remotely.
  • Collaborate to design Reablement Plans and Goals, emphasizing strength-based practice and outcome achievement.

Being co-located with Islington Reablement enhances opportunities to shape Reablement Plans from the outset. You will work integrally with social work colleagues, the hospital, and community services, as well as the Integrated Front Door and First Point of Contact teams in Islington.
