Who You Are

You are a dedicated social worker with a passion for supporting individuals with learning disabilities. You have strong experience in managing caseloads, offering expert guidance, and contributing to integrated care teams. You are adept at working collaboratively with other professionals and have a keen understanding of safeguarding practices and the Mental Capacity Act. Financial competency and a methodical approach to case management are essential to your work.

What the Job Involves

The role involves facilitating supported self-assessments, enabling service users to make informed choices about their services. You will conduct assessments of risk, need, and capacity, and support service users in identifying community services. The position requires you to work with various professionals and agencies to meet user needs, ensure compliance with relevant policies, and maintain high-quality data records. You will manage complex cases, develop support plans, and respond to safeguarding concerns, complaints, and compliments professionally and timely.
