What the Job Involves
Your role involves implementing and supporting individual person-centered care plans, working to improve daily living skills, and participating in assessments and reviews. You will work as part of an integrated team, liaise with other professionals, and support service users in various activities and recreational pursuits. Keeping accurate records and reporting significant changes are key, as is adhering to City Council policies and health and safety legislation.
